The April 2019 issue of The Rx Consultant (New Drugs 2018) included a section that highlighted calcitonin gene-related peptide (CGRP) antagonists, a new class of drugs developed for migraine prevention (erenumab [AimovigTM], fremanezumab [AjovyTM], galcanezumab, [EmgalityTM]). We would like to provide some additional safety information. CGRP is a vasodilator that may help protect organs against ischemic injury; thus, CGRP antagonists might, theoretically, increase the risk of cardiovascular (CV) events (eg, heart attack, stroke) in people with pre-existing CV compromise.< Migraine itself is a risk factor for stroke and heart attack. Currently, there is no clinical evidence that CGRP antagonists increase CV risk. However, the placebo-controlled trials were short-term (3-6 months) and excluded patients with high CV risk. In an ongoing open-label extension study of erenumab, interim 3-year analysis found lower-than-expected CV event rates; however, the expected rates were derived from a large claims database9 (likely including higher-risk migraine patients)...
